You must complete this Return Form within 30 days of the date you received your package in order to receive your Return Number.

Return items must be handed over to UPS within 30 days of the date the package was delivered to you.

We reserve the right to refuse returns that are shipped after the 30-day grace period has expired or items that are not in the same condition you received them in.
